Shopify return form template: give the parcel a clear paper trail
A return form helps staff connect an incoming parcel to the original order. It should support your return policy, not invent one.

- Original order number and order date.
- Customer or company name and a suitable return address reference.
- Item descriptions, SKUs and quantities.
- A blank or clearly labelled reason area when your process uses one.
- Instructions for where and how the parcel should be returned.
Do not promise an outcome the form cannot decide
A printable form does not approve a refund, determine eligibility, create a shipping label or modify the Shopify order. Use careful language such as “return request” or “include this form with your return” unless your actual policy guarantees a particular outcome.
Keep the reason useful and proportionate
A short list—wrong size, damaged, not as expected, incorrect item, other—can help staff route a return. Avoid collecting sensitive explanations you do not need. If photographs or support approval are required, direct the customer to your existing support process rather than trying to embed it in a static PDF.
Connect it to the fulfilment workflow
The item identity should match the packing slip and the original order. For partial returns, make the returned quantity explicit. For exchanges, state the requested replacement separately so warehouse staff do not mistake it for the original shipped quantity.
Print-test the form
Leave enough whitespace for handwriting, confirm long titles wrap, and make sure the instructions remain with the item table on a sensible number of pages. If the form is included in every parcel, test both an empty reason field and a long multi-line order.
